Type of Questions on a Form

Questions

The first thing that we need for each form is a list of the questions. There are many types of questions that we can ask in a form and so we also need to know the type of question and then answer options if the question is a multiple-choice question.

Below you will find some examples of different types of questions:

Short text question: (For example name)

Paragraph text question: (For example tell us about your experience)

Please pick the color that best describes you: (For example radio buttons, they can only pick one)

  • Red
  • Blue
  • Green
  • Yellow

Pick all of the items that interest you: (multi-select - can choose more than 1)

  • Books
  • Movies
  • Hiking
  • Biking
  • Swimming

I agree with the following. (checkbox)

Pricing (If applicable)

What type of pricing do you have for your form? There are many different ways that pricing can be done and forms can have multiple prices within the same form.

  • Single price (there is just one price and there is not a quantity option. This means that every single person has to register for themselves and they cannot register 2 people at a time.
  • Single price multiple quantities (For example, you can purchase two bracelets). Note that if you have a question like this, it may impact the other questions. For example, if you are selling personalized bracelets, then you will need a question for the name on Bracelet 1 and the name on Bracelet 2.
  • Multiple Pricing Options - An example of this is an event where there is a member price, a non-member price, and a student price and they select one of those options.
  • Add on options - An example of this is the exhibitor who pays for their booth and then adds on $50 to add electricity to their booth.
  • Are tax and shipping charged? If so, special consultation is needed on how to handle it.
