Create your own form

Now to create your online web form, I need the following information from you:

  1. Questions - Please list out all of the different questions that you want on the form.
  2. Question type - For each of the questions, tell us what type of question it is (radio, vs. single line of text)
  3. Conditionals - If there are conditional questions, please make note of which questions are conditionals and tell us when they show based on other questions answer.
  4. Pricing - If pricing is involved, please give us detailed pricing. Brochures are helpful if you have them. 
  5. Confirmation - Text of the confirmation message. (The text that shows on the screen after they submit. 
  6. User email notification - Any special email content that goes to the user in the email after they click submit. Do you want to show all of the fields that they filled in on their form? And do you want additional text?

Example: We are so glad to see your interest in volunteering with our non-profit. For your records, please find your volunteer application submission below. (and all of their answers show up below)

    7. Additional email notifications - For each and every notification you send, please give us the email address of the recipient and the text that you want in this email notification. These can also include all of the user-submitted fields in addition to a special message to the person.

Example: This person just completed the registration for the upcoming event, please mark them as paid in Quickbooks. Then the email will list the Name, Email, and all of the other fields that the user filled in. 

IMPORTANT NOTE - Web form creation takes time and adequate user testing. Advance and complex forms with a lot of questions and with conditionals can take a couple of weeks to properly develop and test.
